Hearing Aid Program
Eligibility Requirements:
 
Louisiana residents, age 50 or older,  with a documented hearing loss of 40 dB or greater in the better ear, and an identification card or driverís license.
  
Consumers must provide documentation of meeting the Louisiana Commission for the Deaf (LCD) income criteria, i.e. no greater than 250% of the 2002 U.S. Department of Health & Human Servicesí Poverty Guidelines.

Due to the high volume of requests and to assure the best quality service possible, appointments made in advance are preferred.
 
Consumer Responsibilities:
  
Make an appointment.
  
Notify ABL within 3 weeks of receipt of hearing aid, if any problems occur with function or fit.
  
Complete a Consumer Satisfaction Survey and a hearing aid vendor product information form upon receipt of the hearing aids.
  
  

ABL provides services to the following parishes:

Acadia, Evangeline, Iberia, Lafayette, St. Landry, St. Martin, St. Mary, Vermillion
