to Louisiana residents with a limit of one
item per household from each category.

documentation of sensory impairment may include notice from: Medical Doctor,
Audiologist, Speech/Language Pathologist, Speech Therapist, or Rehabilitation
Counselor; or an audiogram; or a notice on official letterhead from an
appropriate representative (i.e. Council on Aging or senior residential
under the age of 18 must have a parent or guardian’s signature as agreement of responsibility
for loaned equipment.
Due to the high volume of requests and to assure the best quality service possible, appointments made in advance are preferred.
